Freedom of Information and Protection of Privacy
Beginning January 1, 2012, all Ontario hospitals are subject to the Freedom of Information and Protection of Privacy Act (FIPPA).
FIPPA has two main purposes:
It is important to note that personal health information (health records) continues to be covered by the Personal Health Information Protection Act (PHIPA) and cannot be requested under FIPPA. If you would like access to your medical records please visit or call Health Records.
Directory of Records
The Freedom of Information and Protection of Privacy Act requires Manitouwadge General Hospital to maintain a current Directory of Records and Personal Information Banks.
The Directory of Records describes the types of General Records held by the hospital and the Personal Information Banks describe personal information held by Manitouwadge General Hospital. Personal information includes any information that may identify an individual.
Making an Access Request
You can make a formal request under FIPPA to Manitouwadge General Hospital by completing an Information Access Request Form.
Although you are not required to use this form, we recommend that you do so to ensure your request is processed as quickly as possible.A written request referring to the Act will be accepted. Each formal Request Form and /or written request must be accompanied by a $5.00 non-refundable application fee (payable to Manitouwadge General Hospital).

Manitouwadge General Hospital is obligated under the Act to provide you with a decision with respect to your request within 30 calendar days from the date your request is received. This response will provide access to the requested record, or notify you that the information cannot be released along with the specific provisions under the Act preventing release. If a time extension is needed, you will also be notified within the 30 calendar days of your request being received.
Fees
Additional fees may apply based on the size or scope of the request. You would be provided with an estimate and additional information in this event. If the estimated cost is over $100.00, Manitouwadge General Hospital will require a deposit of 50% before performing further work on your request.
