The Family Health Team offers appointments to assess and manage people with chronic diseases, which include:

  • Chronic Obstructive Pulmonary Disease (COPD)
  • Dyslipidemia (abnormal cholesterol)
  • Hypertension (high blood pressure)

The care team may consist of:

  • Registered nurses
  • Registered dietitian
  • Primary Care Provider

Through this program, the patient in conjunction with the care team will set goals and interventions to help the patient achieve optimal health within their disease. The registered staff members are also able to communicate with the patient’s primary care provider to further optimize their care.

Medical directives are in place for the registered staff to order laboratory testing to further assist in disease management. These tests are reviewed by your primary care provider and your chronic disease management team.

Appointments for chronic disease management are typically 30-60 minutes in length depending on the individual’s care needs.

These appointments may include, but are not limited to:

  • Height Assessment
  • Weight Assessment
  • BMI Assessment
  • Blood pressure check
    • 24hr blood pressure monitor (if indicated)
  • Blood glucose check
  • Lung Assessment
  • CAT Score for COPD symptoms
  • Diet Discussion
  • Exercise Discussion
  • Lifestyle Discussion
  • Medication Discussion
  • Goals
  • Referral for laboratory testing

Patient centered goals and working with the health care team to achieve them can help patients achieve optimal health within their disease limitations.
