The Diabetes Program offered through the Manitouwadge Family Health Team is for patients with a diagnosis of diabetes or pre-diabetes. Through this clinic, the registered staff can help patients achieve optimal glycemic control.

The diabetes team consists of:

  • A registered nurse
  • A registered dietitian
  • Diabetes Educator (may be a registered nurse or registered dietitian)
  • Primary Care Provider

The registered staff members are able to make recommendations for lifestyle changes (diet and exercise), set patient centered goals and communicate with the patient’s primary care provider about recommendations for medication changes. The registered staff are also able to order laboratory testing to help monitor patient progress. These laboratory results are reviewed by the patient’s primary care provider and the diabetes team.

Appointments are typically 40 minutes in length but can vary depending on the patient’s individual care needs.

Diabetes management appointments may include, but are not limited to:

  • Height Assessment
  • Weight Assessment
  • BMI Assessment
  • Blood pressure check
  • Blood glucose check
  • Diet Discussion
  • Exercise Discussion
  • Lifestyle Discussion
  • Medication Discussion
  • Goals
  • Referral for laboratory testing

An annual foot exam is also included in this comprehensive program.

The health care team working together with the patient is the key to success in this program!
