The Manitouwadge Family Health Team and the Manitouwadge Physicians Group work together to offer the residents of the community a comprehensive Memory Clinic.

The Memory Clinic is to help identify early onset of cognitive impairment. This is a multidisciplinary approach to cognitive impairment and addresses the needs of the patient as well as their caregivers.

The Memory Clinic team consists of:

• Registered nurse
• Social Worker
• Physician

Through various cognitive assessment tools, the team is able to help identify which areas of the brain may be affected and rule out any reversible causes of cognitive impairment.

If true cognitive impairment is identified, the team will develop a plan of care in partnership with the patient and their caregivers and offer interventions to help slow down further cognitive impairment.

Patients can be referred by their primary care provider, their caregivers and they can self-refer to the program.

Initial appointments with the Memory Clinic team are quite comprehensive and can take up to 2 hours in length. Subsequent visits can vary from 30-90 minutes in length depending on the patient’s/caregiver’s needs.
