Mi Card (Medical Information Card)

Your personal Mi Card is a handy reference to keep in your wallet and can be used in a medical emergency.

A registered nurse will assist you in creating your individualized card.

Please have the following ready:

• Medical diagnosis’
• List of medications
• Allergies
• Your family doctor’s name
• Name of any other doctor’s that are currently treating you
• Emergency contact information

You can also complete the form online and email it to infoserv@mh.on.ca or print off the form and mail it in to;
