Senior Service Van

Santé Manitouwadge Health in partnership with the Township of
Manitouwadge is pleased to provide accessible transportation services for
seniors age 65 and older and for residents who require accessible
transportation within the township of Manitouwadge.

Service

The myRide program offers transportation services for eligible seniors
and persons who require accessible transportation to attend medical
appointments, health related appointments, and wellness events.

Eligibility

If you are a senior age 65+ or an individual who requires accessible
transportation, you are eligible for the service and can register. You can
register either by coming to the Family Health Team and pick up an application
or by calling one of our coordinators at 1-807-826-3251 Ext 130 for more
information.

Payment

Clients can purchase rides through the Family Health Team reception desk
at Santé Manitouwadge Health.

Hours of Service

The myRide program is available for transportation Monday to Friday
8am-4:30pm. Evening and weekend services can be arranged by appointment only.

Booking

Clients are encouraged to book their rides at least 24 hours in advance
of their appointment. For weekend rides, booking must be completed by noon on
Friday. Please call 826-3251 Ext. 130.

myRide Program

The myRide program is delivered by Santé Manitouwadge Health in
partnership with the Township of Manitouwadge.

When is it Used?

The service is NOT just for medical appointments. You can book the
myRide van for ANY trip within Manitouwadge – going shopping, attending Golden
Age Center activities, church services and more!

Who is Eligible?

• Any resident of Manitouwadge who is 65+
• Persons of any age who require wheelchair accessible transportation
• Persons who have physical limitations that cannot get in\out of a regular taxi
• This includes those that may require the service temporarily while recovering from surgery/injury and cannot use a regular taxi

Rates

Rides are $2.50 per ride or $5.00 round trip.
Registered caregivers can join clients free of charge.
