Santé Manitouwadge Health has partnered with the Northwest Local Health Integration Network’s (NWLHIN) Home and Community Care Program to offer nursing home care services in the community. The Family Health Team registered nursing staff are responsible for delivery of this service. Care is only available by orders received from the NWLHIN Home and Community Care office.

This service may be beneficial for the patient who is transitioning back home from hospital and/or for patients who are physically unable to attend out-patient care services.

Patients can be referred for nursing home care services by their primary care provider. Patients can also contact the NWLHIN Home and Community Care office in Thunder Bay and start the self-referral process to determine if they are eligible.
