What is Respite Care?
A pre-planned admission for the chronically ill adult to provide relief for caregivers. Santé Manitouwadge Health is the only facility providing respite care to our community.
Who is eligible?
Preference will be given to adults who require complex continuing long term care level of care. Applicants who would be disruptive to other residents, risk of wandering/exit-seeking, risk of harm to self and others at our facility may not be considered.
How to apply?
Caregivers are required to contact their doctor to discuss their needs. The doctor will then submit a Respite Request Form a minimum of four (4) weeks in advance. Santé Manitouwadge Health will contact the caregiver to give notice of acceptance or denial of the request.
When will respite be provided?
Respite care will be provided to only one patient at a time.
Each patient requiring respite care will be limited to four (4) weeks care per year.
Each respite admission will not exceed two weeks.
Scheduled admission of respite care patients will be based on availability of beds and appropriate staffing levels to meet the needs of the patient.
Once admitted if the bed is required for acute care admission the patient may be discharged to the community.
How much does it cost?
The cost for respite care is based on the daily rate set by the Ministry of Health & Long Term Care Hospital Chronic Co-payment.
Additional respite services can be coordinated through the Community Care Access Centre/Patient Coordination Services. Please call toll free 1-877-661-6621.
