The Manitouwadge Family Health Team social worker offers Service Navigation appointments. These appointments are to help patients source out potential services they may benefit from, apply for these services or apply for funding for services/devices/programs.

Patients are advised to bring any pertinent forms and information to these appointments. Patients are encouraged to review the forms prior to their arrival to ensure they have all of the necessary information. This may include such things as a social insurance number, current medical insurance, banking information, spousal information and any other sources of funding they may be receiving. If you are unsure what to bring, please contact the social worker in advance at (807) 826-3251 Ext. 277 to verify any information that may be needed.

Appointments are scheduled for 60 minutes in length. There may be times where multiple appointments are required. This can be determined after the initial appointment.
