The Manitouwadge Family Health Team in partnership with Centre for Addiction and Mental Health and Moving On To Being Free delivers a free Smoking Cessation Program.

The Smoking Cessation Program is a 12 month program that offers education, support and free nicotine replacement therapy to help a patients succeed in becoming and remaining smoke free.

Initial appointments are scheduled for 60 minutes with a care provider. All other appointments throughout the year are telephone calls to monitor progress and offer support. The program offers up to 26 weeks of free nicotine replacement therapy which includes:

• Nicotine Patches
• Nicotine Lozenges
• Nicotine Gum
• Nicotine Inhalers

The NRT does not have to be used in 26 consecutive weeks, the patient has up to 12 months to use up to 26 weeks of free NRT.

PLEASE NOTE: There is a package to be picked up and completed by the patient prior to their initial appointment. Failure to complete the required forms in advance may result in the appointment being rescheduled. Patients are able to opt out of the program at any time.
